Complete a Patient Emergency Planning Needs Assessment

Objective: To assess patients’ level of preparedness for an emergency or disaster.
Activities
Required:
  • Conduct a Patient Needs Assessment with 90% of patients in the facility.
  • Have patients initial that they have completed the assessment.
  • Submit completed Project Reporting Form.
