framework for disaster readiness and continuity of care for the renal community

Provide Patient Resources for Disaster Preparedness

Objective:	To increase patients’ knowledge so that they are better prepared for emergencies or disasters.
Activities Required:	Provide patient education on preparing for emergencies or disasters (at least 3 resources per patient). Document the provision of resources. Submit completed Project Reporting Form.
